Order Form

1. You can simply download the PDF order form below, fill it out and provide as much detail as possible.  Or if you have a form/list that you prefer to use then feel free to use that as long as you have the dimensions (width x height x depth) and materials preference.    
2. You can then email it to info@jcscreations.ca to get the ball rolling.
3. We will respond within a few days and determine if we require more information to complete the quote.    
4. A 50% deposit is required at time of order and the balance is due on delivery for all orders, special requests are reviewed.   
5. We accept E-transfer, cash or cheque for payments.   
6.  Approximately 1-2 weeks prior your order completion we will touch base to finalize colour and any further details and date of completion.
7. The week of your order completion we will touch base to arrange pickup time or delivery.
